# Every finding, tracked to a decision

Clearly AI turns review findings into risks, each one grounded in your policies, scored with your methodology, and assigned to an owner. Open a ticket for every risk, and the registry updates as the work gets completed. Product security, privacy, and GRC work from the same record.

## What happened to that finding?

The review is done and the findings are in the doc. Then someone copies each one into Jira. Someone else re-scores it for the GRC register. Privacy logs the same issue in their own system. A month later, nobody's sure which tickets closed, which risks were accepted, or who signed off.

So the security team chases owners in Slack and checks tickets by hand. And when an auditor asks who accepted a risk and why, the answer is buried in a thread.

## How the risk registry works

Every risk starts from a finished review. Here’s what happens next.

1. **The review surfaces the risks**

When a review finishes, Clearly AI identifies the threats, control gaps, and risks it found. Each one is tied to your policies, linked to the source behind it, and shows an AI confidence score. Your team chooses what gets published to the registry.

2. **Clearly AI scores each one your way**

Each risk maps to an entry in your risk catalog. Severity comes from your methodology, the fix-by date comes from your SLA, and suggested remediation is ready for the owner.

3. **Your team decides how to treat it**

Accept it, mitigate it, transfer it, or stop the launch. Each decision is recorded with who made it, when, and why.

4. **The registry tracks it to close**

Clearly AI opens a ticket for each risk in Jira, Linear, or ServiceNow. When the ticket moves, the risk moves with it.

## Only the risks that apply to what you’re shipping

A risk registry should hold the risks that matter for the system under review, measured against the policies, controls, and standards your company actually uses.

- **Grounded in your policies:**
Control gaps are measured against your own policy library and frameworks
- **Specific to the product:**
Risks reflect the system's architecture, data flows, and dependencies, drawn from its design docs, tickets, and code
- **Linked to the source:**
Every risk links back to the review and the answer it came from, so reviewers can check the reasoning before they publish
- **Scored for confidence:**
Each risk carries an AI confidence score. Low-confidence risks are left unselected by default, so your team reviews them before anything reaches the registry.

## Put your team’s risk standards in one catalog

Every security team has risks it sees in review after review, and a standard way of handling most of them. Put that knowledge in a risk catalog, and Clearly AI applies it to every new risk.

- **Build your risk catalog:**
Define common risks with a risk statement, category, default severity, suggested controls, and mapped frameworks. Start from scratch or import a CSV.
- **Score with your methodology:**
Use an impact and likelihood matrix or a severity-only model, with severity levels and fix-by SLAs that match your policy
- **Set the remediation:**
Configure suggested controls and action items for each catalog risk, or let Clearly AI suggest remediation guidance tailored to the system under review
- **Keep it consistent:**
Catalog entries are versioned. Each risk keeps the version it was linked to, so the record still holds up even as your standards change.

## Four ways to treat a risk. Every decision on the record.

Some risks get fixed. Others get accepted, reassigned, or stop a launch. Clearly AI supports all four standard treatments and records the decision behind each one.

- **Accept:**
Accept the residual risk with a written justification and residual risk notes. The decision is saved with who made it and when.
- **Mitigate:**
Track remediation against a plan, an owner, and a fix-by date set by severity
- **Transfer:**
Reassign the risk to the team or party accountable for it, like a vendor owner or another business unit
- **Block:**
When a risk is too high to accept, halt the launch and record why

## A registry that updates as engineering works

Clearly AI opens a ticket for each risk in Jira, Linear, or ServiceNow, with the finding, severity, fix-by date, and suggested remediation already filled in. Two-way sync keeps them matched. When an engineer updates or closes the ticket, the risk in the registry updates too.

Security doesn’t have to check tickets by hand or ask in Slack whether the fix shipped. The registry shows where every risk stands.

## One registry for security, privacy, and GRC

The same risk shouldn't live in three systems with three different scores. Product security, privacy, and GRC can work from the same registry on the same license. Filter by project, category, or catalog risk, and see the decisions other teams have made.

Already have a system of record? Send risks to the registries your organization runs, like Wiz, ServiceNow, and Archer, so your enterprise risk view stays complete.

## Why not just track risks in a spreadsheet?

A spreadsheet or ticket queue can hold a list. Clearly AI keeps the risk connected to the review that explains it.

- **Risks start from the review:**
Each risk is created from a sourced finding, so nobody has to re-type it from a doc
- **The standards are yours:**
Your catalog, scoring, SLAs, and remediation guidance apply to every risk
- **Status comes from the work:**
Ticket sync updates the registry, so it doesn't depend on someone remembering to
- **The record holds up in an audit:**
Source, severity, owner, treatment, and every decision are in one history

## Coverage that holds up after the review

- **~90%** Reduction in time per review
- **From 10 to 100%** Product coverage

_FAQ_

## Answers to your top questions

### What is a security risk registry?

A security risk registry, or risk register, is a system of record for risks that need follow-up: what the risk is, how severe it is, who owns it, how it will be treated, and when it’s due. In Clearly AI, risks are created directly from security, privacy, and vendor reviews, so each one links back to the finding and sources behind it.

### How does Clearly AI decide what counts as a risk?

When a review finishes, Clearly AI identifies threats, control gaps, and risks measured against your policies and relevant to the product under review. Each candidate risk shows an AI confidence score and a link to its source. Your team chooses which risks to publish to the registry.

### What is a risk catalog?

A risk catalog is your library of common risks and how your team handles them, including a risk statement, category, default severity, suggested controls, and mapped frameworks. Clearly AI maps new risks to catalog entries so they’re scored and remediated the same way every time. Build the catalog in Clearly AI or import it from a CSV.

### Can Clearly AI use our own severity scoring?

Yes. Configure an impact and likelihood matrix or a severity-only model, and set fix-by SLAs for each severity level. Clearly AI calculates severity and due dates with your methodology.

### What risk treatment options does Clearly AI support?

All four standard treatments: accept (tolerate), mitigate (treat), transfer (reassign), and stop the launch (terminate). Each decision is recorded on the risk with who made it, when, and the justification.

### Does Clearly AI sync risks with Jira, Linear, or ServiceNow?

Yes. Clearly AI can open a ticket for each risk in Jira, Linear, or ServiceNow and keep them in sync both ways. When the ticket is updated or closed, the risk in the registry updates automatically.

See the Jira integration

### Can we send risks to Archer, ServiceNow, or Wiz?

Yes. Clearly AI can feed risks into the registries your organization already uses, like Archer, ServiceNow, and Wiz, so your enterprise risk view stays complete.

### What does the AI confidence score mean?

The confidence score shows how strongly the review’s sources support a risk. Low-confidence risks are unselected by default when you publish, so your team reviews them before they reach the registry.

### Is there a record of who accepted a risk?

Yes. Accepting a risk requires a written justification, with optional residual risk notes. The decision is saved on the risk with who made it and when, alongside its full history.

### Does the risk registry replace our GRC tool?

Only if you want it to. Many teams use Clearly AI as the source of review-driven risks and send them to their GRC platform. Others use the Clearly AI risk registry as their primary record for product and privacy risk.

### Does the risk registry continuously monitor our systems?

No. Clearly AI doesn’t watch your systems or dashboards. Risks come from point-in-time reviews of a specific project version, and their status updates from the tickets linked to them.
